Veronica becomes class parent of the day at Amy and Gemma's school, but is horrified on the type of protocols and drills that they take the kids through due to COVID-related budget cuts. Frank ends up in the hospital after his continued confusion lands him in a fight with old marijuana suppliers; the doctor then informs him that his prolonged amnesia is due to early stage alcoholic dementia. Carl meets a cashier named Tish on his day off; after a day of courting her, he takes her back to his place, where she forces him to have sex with her without a condom. Debbie then informs him that he was date raped.

Lip tries to have a family meeting on whether to sell the family home because he isn't satisfied with the alternative of moving in with Tami's parents. He is only met with resistance from Debbie. Kevin decides to take the stolen bikes that Lip is hiding in KevFit for a spin, but when he gets pulled over by the cops, he panics on how to get rid of the evidence. Frank tries to use his diagnosis to his benefit, but realizes that his situation is beyond his control. Veronica finds out that her mom is selling her house and moving to Louisville, so she tries to convince her to stay in Chicago because of the migration of black people due to gentrification. Debbie finds out that Sandy has a son with her estranged husband, and they get into an argument because Sandy says she wants nothing to do with her son. Carl is paranoid that Tish tried to conceive a baby with him so that she can tap into his job benefits. He confronts her with a morning-after pill and a bottle of water, only to learn that she is independent, strives to be a dental hygienist, is allergic to latex and was on the contraceptive pill at the time of their tryst. Ian gets self-conscious when Debbie says that he only wants to sell the house because Lip wants to, as Lip is one of his only friends according to Mickey, so he convinces him to go out and make gay friends as a couple. After a day of sexual escapades, he realizes that he is content with the friends he has currently.

Ian and Mickey are tasked to find a caretaker for Terry since his brothers and cousins abandoned him, but after Terry keeps running off applicants due to his blatant racism, they hire a nun to fill the position. After Mickey worries about his dad's safety, he runs home to see that the nun has suffocated him to death. Due to the Carl Curse, nobody wants to partner with him on the force, so the Chief puts him with the vice squad. He finds out their crooked dealings are to put a stop to legal marijuana. Frank opposes efforts to rename Liam's school, named after former Speaker of the U.S House of Representatives Dennis Hastert, due to Hastert's molestation conviction. 1319"Survivors"Iain B. MacDonaldJoe LawsonMarch 21, 2021 (2021-03-21)0.48[11]

Veronica follows her Mom to Louisville to convince her to move back to Chicago, while Kevin is left in charge of Gemma and Amy at the Alibi, which proves to be too much for him. Frank tries to reconvene his old heist buddies for one more caper, but realizes that all of them are either dead, retired or disabled. Lip and Brad find out that the new owners of BornFree are one of the biggest crime families in Chicago; they are kidnapped by the boss to fix his son's mini Mercedes. After rummaging through Terry's stuff, Ian and Mickey find memorabilia from an old flame of his. After tracking her down, they learn that her family is Jewish and Terry wanted to marry her, but he killed her father after he wouldn't lend her hand in marriage. She went on to marry a Black man, which likely explains much of Terry's bigotry. They end up burning the memorabilia along with his body. After his "accident" with the Vice squad, Carl gets relocated to the Eviction Unit, with his old partner, Officer Tipping, and also finds discrepancies with this department as well. Since no one has reassured him of where he will be living, Liam tries to find other places to live, but after he contemplates foster homes, group homes and even incarceration, he ends his search after Lip steps up to the plate.
13210"DNR"Anthony HardwickCorina MartescuMarch 28, 2021 (2021-03-28)0.59[12]
The entire Chicago police department is looking for the Nighthawks painting that Frank stole from the Art Institute of Chicago, so the family leaves Liam in charge of Frank to make sure his dementia doesn't cause him to say something to implicate them. While Liam tries to prevent a student from taking his spot at a STEM school, he and Frank discuss how Frank's diagnosis will affect the family. Debbie tries to look for a new place to stay after they sell the house, but gets depressed due to the family bonding she will miss when she moves. Kevin and Veronica help her mom move into her house in Louisville, but after seeing the opportunities and people there, they consider moving there themselves. Lip continues to fix the house to increase its value, but after the costs are revealed, he must make tough decisions in order to meet that goal. Ian and Mickey find a cheap luxury apartment, but they argue because Mickey does not want to move to the West Side. Ian signs the lease without Mickey's consent. Carl and the police department come to a head when he sees how badly they treat poor people. After Franny puts stickers all over the painting, Ian and Lip leave it on the subway, where a homeless man finds it.
13311"The Fickle Lady is Calling it Quits"Iain B. MacDonaldNancy M. PimentalApril 4, 2021 (2021-04-04)0.52[13]
Ian and Mickey have different experiences in their new apartment in the West Side. Ian enjoys meeting his new neighbors and using the amenities, while Mickey has a hard time adjusting from the loud and chaotic vibe of the Southside to the quiet and friendly one on the Westside. Ian convinces him to try to make a better effort to get acquainted to his new home. Kevin and Veronica prepare to sell the Alibi before they move to Louisville, and they find old memorabilia that take them down memory lane. Carl is demoted to a meter maid after refusing to apologize for his assault incident, so he uses his new "power" to stick it to the rich and look out for the poor. Debbie gets worried that Frank and Monica's bad parenting have made it impossible for her to find lasting love, until she runs into a recently released ex-convict named Heidi. Lip gets an offer to sell the lot where the Gallagher house is, but after he tries to bargain for a higher price, the buyers cancel the offer after he has already begun selling items from the house. Liam and Frank spend the day doing "Frank things"; as his dementia continues to worsen and he sees it causing more problems other than memory loss, Frank decides to overdose on heroin.
13412"Father Frank, Full of Grace"Christopher ChulackJohn WellsApril 11, 2021 (2021-04-11)0.70[14]
The family finds Frank unconscious on the couch after his latest overdose, but are unfazed due to this being a regular occurrence. Debbie and her new fling, Heidi, start to get more acquainted, which leads to her tagging along in her hijinks and Heidi asking Debbie to run away with her to Texas. Ian and Mickey start furniture shopping for their new place, and while they are rummaging through furniture through Kevin and Veronica's house, Ian says he wants to have kids. At the end of the day, Mickey surprises him with a party celebrating their first anniversary after pretending to forget about it all day. Lip starts to do food delivery after the real estate deal falls through; he gets another offer from a neighbor, but for less than half of what he was originally offered. Kevin and Veronica begin the process of selling the Alibi, but end up unsure if they are going to sell it because the deals are insufficient and they fear that the new owners will change the culture and history of the bar. Carl continues to punish the rich through parking tickets, but it ends up not being as fulfilling as it was in the beginning. Officer Tipping suggests that they buy the Alibi together and turn it into a cop bar. Liam searches for Frank after he wakes up from his overdose and wanders off to places that he is reminded of due to his dementia. After breaking into a church, Frank is taken to the hospital, where he tests positive for COVID-19 and becomes critically ill. He reminisces about his family and his past before dying; Frank's ghost appears at the party before floating off over the city on his barstool. The rest of the family is shown enjoying the party while watching a gentrifier's Tesla go up on fire outside of the bar. In a mid-credits scene, Frank's body is cremated but ends up exploding from the heat due to the large amount of alcohol still in his system.
