Eight months after Kieran's arrest, the new Brandon James slasher murders him and a security guard in the courthouse. The remaining Lakewood survivors are shocked by his murder and, with Halloween being one year since Piper's unmasking, they decide to take a vacation from Lakewood and travel to Shallow Grove Island. There, Stavo, Noah (now published authors, having written a book about the murders of their friends) and their book editor Jeremy plan to research Anna Hobbs—a girl who supposedly killed her mother, brother, and the man having an affair with her mother, Reginald Whitman—in the hopes of writing a new book. Emma meets Alex Whitman, a descendant of the man killed by Anna, and the two take a romantic interest in each other. After a party, Stavo hits on a woman named Billie to make Brooke jealous. After Jeremy returns, alive and unscathed, he is locked in a closet by Alex, as everyone believes he is the killer. However, he escapes and discovers a picture of Billie and the real Alex Whitman, revealing the Alex they know is an imposter and the killer; Alex, whose real name is Tom Martin, then kills Jeremy. Noah, Audrey, Brooke, and Stavo travel through a secret passageway from the mansion to Billie's house. There, Noah deduces that instead of Anna being the killer, she and her family were attacked by Reginald and he was only killed by Anna in self-defense after stabbing and attempting to rape her. Anna later died from her injuries. Emma is briefly attacked by Tom in his costume, but she locks him out of her room. Tom takes off the costume and gets her to let him in the room with her by pretending to be Alex. However, Emma discovers the corpse of the real Alex and realizes that Tom is the killer. Tom explains that his parents were murdered and he, like Emma, is a survivor and he felt it necessary to kill anyone standing in the way of a relationship between the two after seeing her on the news. Tom also reveals he did not kill Kieran and does not know who did. When Emma explains that she does not love him, he chases her until she shoves him off the balcony of the mansion to his death. Upon arriving at school, Deion finds a TWINS bar (candy) in his backpack with a note that says "Trick R Treat", then later during the day finds another TWINS bar in his locker with a note saying "Parking Garage. After School.". He leaves to find his stalker, but ends up getting into a physical conflict with teammate Avery Collins trying to defend Olivia Reynolds, an exemplary student and cheerleader. As a result, the two along with activist Kym, gothic horror fan Beth, Kym's best friend Manny and musician Amir are put in detention together, being compared to The Breakfast Club. During detention, Deion leaves after receiving a text that says to go to the locker room. When he gets there, he is tasered by Ghostface and left unconscious in the hallway. During detention, the group is invited by former student and drug dealer Shane to a party that night. Later, Deion receives a threatening link from the self-titled serial killer "Ghostface," which threatens to do harm to the detention group, and Deion with no choice goes with Amir to Shane's party only to discover that everyone present there is wearing the Ghostface killer mask. Under pressure from Ghostface, Deion reunites Liv, Kym, Manny, Amir and Beth and shares the story of his twin Marcus, and how he was afraid to help him. But soon after this, Avery is attacked and killed by Ghostface when thrown from a catwalk and is impaled on a wooden stick. After the scare, Beth reunites the Five's Club in her jobs tattoo studio, where they come to the conclusion that they are being pursued by a serial killer and that according to the rules of the horror films, it is unlikely that the group will reach the end alive.

The next morning, Kym reunites with Beth, Manny, Amir and Deion to discuss ways to survive, while Liv is forbidden to leave home by her father, Officer Reynolds. During a lecture in memory of Avery, Kym shares with the group her theory that Shane is Ghostface, because the party he hosted had the same mask that Ghostface uses and Beth creates a plan to attract him, to make him confess it. Liv uses her father's computer and collecting evidence from coroners, she encounters Deion and shares her theory that Marcus is alive and he is Ghostface, but Deion is reluctant to believe. Meanwhile, Beth, Amir, Kym and Manny lure Shane to school, where they leave him unconscious and press him to confess that he is Ghostface, but he says he is also being threatened by the killer, who locks them in high school. The group ends up separating during the escape, which results in Kym being persecuted once again, but is partially saved by Shane. As soon as he arrives at the school to help them, Deion comes face to face with Ghostface who makes the same signal with the fingers that Marcus did, and from that, Deion believes in Liv's theory that Marcus is alive. Deion borrows Liv's car, which with the help of the rest of the group, traces him to an out-of-town gas station. Amir decides to go with Deion and Liv to the location marked by Ghostface, but Liv punctures the tires of Kym's car to keep Kym, Manny and Beth away from danger. However, Kym believes truck drivers did this as revenge because she insulted them earlier to defend Manny. At the location marked by Ghostface, Amir waits in the car while Liv and Deion meet with Luther, the ex-guerrilla Hook Man and owner of the old junkyard where Marcus was attacked. Luther claims he only hurt Marcus that night, who fled and hid in an unknown location. A few months later, Luther found Marcus dead in a trunk of an abandoned car and buried him in an improvised grave. At the station, Kym and Manny have a fight, which results in Kym trying to return alone to the city in her car, only to be chased by the same group of truck drivers who make her lose control of the car and crash inside a cornfield. Deion and Luther are pursued by Ghostface in the junkyard, where Ghostface ultimately kills Luther by chaining him inside a car crusher. Liv looks for Amir, who comes running to the cornfield where Beth is. Manny, who went ahead to look for Kym, is attacked by Ghostface, and hides inside Kym's car. However, he becomes stuck, and Ghostface sets fire to the car, which blows up, killing Manny. Amir and Beth watch Kym in tears over Manny's death, while Deion and Liv listen on the phone, horrified. Later, a traumatized Liv tells Deion her suspicions that the killer is someone in the group.
The title of this episode is a reference to the 1986 song "He's Back (The Man Behind the Mask)", which was the theme song to the 1986 American slasher film Friday the 13th Part VI: Jason Lives.[citation needed]

During Manny's funeral, Kym receives flowers along with a note written by Ghostface, telling her to choose who in the group should die, and stating that "snitches die". Shortly thereafter, the group meets up and Deion decides that no one in the group should respond to the message, so no one will be chosen. At night, Deion asks Jamal for a gun and goes to Liv's house to discuss their next steps, only for them to have sex. Amir goes to Beth's house, where they have a talk about who they really want to be, theorize that Liv can be Ghostface and soon after, they have sex too. Kym does a simple homage to Manny on the roof of the school, where Hawkins, Deion's teammate, shows up and reveals that he and Manny were lovers. Deion finds a private telephone in Liv's drawer, and a receipt of flowers Kym received earlier that day, and when confronted about it, Liv discovers the weapon in his backpack, and in fear, calls her father to arrest him. However, Reynolds takes him into the middle of the forest, where he reveals to Deion that Liv has been mentally unstable since the separation of her parents, and fears that she has personality disorder. During the conversation, Reynolds is attacked and stabbed by Ghostface and Deion scared calls the police, only to discover that they have already been called and flees. The group gets a message announcing that Deion has been chosen to die and Beth discovers that Amir has voted to do so according to what Deion had previously requested. But Beth remembers that the message said "snitches die," and that may indicate that Amir is in danger because he chose Deion. Soon, Ghostface invades Beth's house and they both run through it together but end up splitting up in the process. Amir is killed by Ghostface in the elevator. While the ambulance takes his body, the police arrest Deion on suspicion of attacking Reynolds.

Liv visits her father in the hospital, who is alive, but unstable. Deion is held until the next morning by the police who releases him for lack of evidence, and later is picked up by Jamal. During the ride, Deion discovers that DJ "Mr. Fade" (Shane's former alter-ego) will throw a party that night, and on suspicion of being Ghostface, asks Jamal to let him work at this party, where Jamal reluctantly accepts. Despite the differences, Kym and Beth team up to gather evidence that Liv is Ghostface, but both have separate and enlightening conversations with the girl, who reveals having the private telephone to communicate with her mother, and that she feels guilty for the divorce of her parents. Deion finds Ghostface's mask in Jamal's trunk, and when confronted, he says it is only for use at the party. After Kym tries unsuccessfully to get information from Jamal during the party, he talks privately with Deion and tells him that he himself is Mr. Fade, and at the same time that Deion reveals the existence of the serial killer, Jamal acts in a strange way and insinuates that he is Ghostface, saying he will go after Reynolds and Liv that night. Deion rushes to the hospital where Ghostface has arrived, killed a police officer, and is now trying to turn off Reynolds' life support machines, but Deion saves him, and shares with Kym his discovery about Jamal. Soon, Kym and Beth tell the police about Jamal, who at the place of the now empty party, is attacked and seriously injured by Ghostface, who accuses him of having "taken a forbidden advantage."

On the night of the big game, Deion is convinced that it is not worth playing when a killer is on the loose, but with the police looking for Jamal, Kym and Liv convince him to get what he dreams of doing. Motivated, he returns to the team just in time. Meanwhile, Ghostface keeps a wounded Jamal in an unknown location. While on the way to the game, Beth and Kym receive a video of Ghostface showing Jamal injured and that Deion must go alone to "Earl's house," a truck with that name. Kym creates a distraction and Beth invades the game to show the video to Liv, who tells Deion during the interval of the game. Deion comes to the conclusion that Ghostface wants him because, on the night of Marcus's death, they both changed their fantasies and Deion, in Marcus's clothes, was the one who died that night. So Marcus had always been Deion since then because no one recognized him, not even Sherry, his mother. Soon after the revelation, Marcus steals a school bus and arrives at the place where Jamal is, and takes him away. But out there, Jamal reveals that he is really Ghostface, having taken the mantle because their father chose Deion's mother and Marcus instead of Jamal's mother, and that made Marcus have the perfect life. He also reveals that he has a female accomplice and that she "will end it all," but he dies from the wounds before he reveals who she is. Liv and Beth invade the school where Marcus arrives shortly after. The two girls are accusing themselves of being Ghostface, but before Marcus calls the police, Beth shoots his cell phone and hitting his hip with the bullet, revealing herself as the accomplice of Jamal. They met at her tattoo studio, and shortly after he told her Marcus' story, Beth had realized that she wanted to be the protagonist of her own horror story. With Marcus wounded, Liv rushes through the school to save herself and with Beth in her chase dressed as Ghostface, pursues her with a gun, and soon after with an ax. The confrontation reaches the roof of the school, where Marcus distracts Beth and Liv pushes her from the rooftop into a glass ceiling of the gym. Kym arrives soon after and shoots Beth dead. A few months later, Kym and Hawkins went to college together. Marcus (still using his original name Deion, implying that Beth's theory is fake) attends San Diego University with Sherry and Liv accompanying him, where they enjoy life without worrying about Ghostface and Liv comes to the conclusion that they should not tell the truth about the exchange of twins. At the same time, Deion receives an anonymous call but ignores it.
