Audrey's secret girlfriend, Rachel Murray, is murdered by the killer, who stages it as a suicide. Her death raises questions and causes yet another rift between Emma and Audrey. Meanwhile, Piper, a provocative podcaster comes to Lakewood to report on the town's murders, past and present.

Branson escapes from prison and the hunt for Sheriff Hudson is on. They find Hudson tied to a tree and as Maggie unties him, she accidentally disembowels him and he dies. Brooke, Kieran, Audrey, and Jake are unaware of the killer's grand finale. With no idea where they are, Emma and Noah race against time to save their friends. At the party, the killer injures Audrey and Brooke but they survive. Emma is lured to the dock by the killer, who is holding Maggie hostage. The killer is revealed to be Piper Shaw, Maggie and Brandon James' daughter. She explains to Emma that she should have had the life Emma got, and killed her friends to watch her suffer. After a struggle, Piper tells Emma she has one more surprise before she is shot in the chest by Audrey. Emma then finishes her off with a bullet to the head, killing her. Noah takes over Piper's podcast and talks about the possibility of Piper having an accomplice, noting the night her and Will were attacked. Audrey is then shown burning letters she had exchanged with Piper months prior to the murders.

As all returns to normal, Audrey is pranked by Haley Meyers and another stranger attempting to terrorize her and ends up stabbing the other stranger because he was dressed as the previous killer and wearing a Brandon James mask, who survives since his camera strap blocked it. Emma returns home after three months away at a treatment facility for her PTSD. Brooke breaks up with Jake after he angers her. The same night, Jake is secretly attacked by a new killer. Noah begins an obsession with the previous murders and theorizes that Piper had an accomplice, which drives feelings of unease with Audrey considering her former connection with Piper. Audrey is also harassed by someone who is aware of her interactions with Piper. Emma notices a picture of a barn on Noah's murder mystery board, to which she makes a link of the place in her dreams. Curious, she and Brooke go to investigate the barn. There, Jake is murdered with a scythe but it goes unnoticed by Emma and Brooke. Emma enters the barn and finds nothing, causing her to question why she is dreaming about it. She then turns to see someone living in the abandoned house next door. Emma enters the house seeing pictures of her younger self and clippings of the murders. She then notices a dark figure in the background and screams.

Emma, who escaped the barn unharmed with Brooke, returns with Maggie to find nothing Emma saw earlier there anymore, concluding she hallucinated. Kieran's aunt, Tina, says that Kieran has to move to Atlanta because he is living in his dad's house without a guardian. Emma turns to Ms. Lang to talk about her PTSD and has a hallucination of Piper. Noah finds something that leads him to a motel where the receptionist, Eddie, claims he saw Piper's accomplice. Emma faces her fears at the dock at Wren Lake before getting followed home by someone who is revealed to be her father, Kevin, and they agree to meet for breakfast the next morning. Audrey is led to an abandoned warehouse where she finds Jake's body.

Audrey is shocked when she finds Jake's body, and gets a call from the killer, forcing her to flee the scene. After Emma and her father have an argument in the coffee shop, she leaves while Kevin books himself into Crescent Palms Motel. Kevin tells Maggie that he has been receiving emails from Riley Marra, which scares Maggie since Riley was murdered by Piper. At the motel, Eddie is murdered with a corkscrew by the killer in Kevin's room. Emma goes to Crescent Palms and tries to apologize for storming out earlier but finds Kevin's room empty. The killer video chats Audrey a message and threatens to kill Emma. Audrey calls the police anonymously but finds Emma outside confronting her father over a drunken fight. Emma finds herself not being able to trust her father when he plans to leave Lakewood again without telling her. Audrey lies to the police about the reason she called them. Emma's father reveals the truth about a past incident when Emma was young. Disgusted, Emma walks away with Audrey. Audrey then gets in her car while the killer sends her another message stating to look under her car seat where she finds the bloody corkscrew the killer used to kill Eddie.

Audrey attempts to bury the bloody corkscrew but the killer recovers it and she finds it in her bed the next morning. Emma has a bad dream of Kieran getting stabbed by the killer. She later texts Kieran to say happy birthday. Against Kieran's will, Eli tells Emma that Kieran wants a surprise party which leads them to organize it at Emma's house. Sheriff Acosta begins to worry about Jake's absence. Audrey finds strange drawings of the Lakewood Six on Gustavo's iPad. At the party, Eli and Emma almost kiss and a bottle of tequila by "Jake" causes everyone to fall ill and start hallucinating. Audrey hallucinates Rachel, making her kiss Noah. Emma encounters the killer in the woods, but since it is unclear whether or not it was a hallucination, she is disbelieved. The next day, Eli apologizes to Emma and in assembly, the Lady Of The Lake finalists are announced which leads to blood and guts being dropped on Brooke. Then Jake's body falls from the banner and Brooke begins screaming as the audience looks on in shock.

After Jake's body falls in the theatre, Sheriff Acosta puts the school into lock down. Emma and her friends are then trapped inside the school with the killer. Emma finds Jake's phone in her bag, while everyone questions who put it there. When the authorities begin searching the students' lockers, Kieran and Noah sneak off to get Kieran's bag, which has his gun inside. Kieran is caught by Deputy Dwayne, while Noah is stuck in the hallway. Kieran tells Sheriff Acosta about Jake's phone in exchange for not being expelled due to the gun in his locker. Emma comes back from questioning, confronts Kieran and sees that Haley has been talking about her behind her back. Emma confronts Haley, which ends in a brief fight between them. Ms. Lang takes Emma to a sleeping bay where Emma is attacked by the killer after Ms. Lang leaves and locks the door. Emma escapes by breaking the glass with a chair and the killer disappears. Back in the library, Audrey watches Gustavo for a while before Noah comes through the Emergency Door and confronts her on why she has been acting strangely. She walks towards Gustavo and takes his iPad, showing everyone his graphic sketches of the Lakewood Six. Gustavo is beaten by the other students, before the fight is broken up by Acosta. When the school officially opens, Noah steals Audrey's phone. On the phone, Noah sees Audrey with Jake's body in the warehouse.

Emma and Noah investigate a tip from Eddie at the local amusement park. Noah and Audrey are kidnapped, and Audrey confesses to Noah about her encounter with Jake's dead body. Audrey is revealed to be behind their kidnapping, which she orchestrated to prove she is not the killer. Emma runs into the killer before Audrey and Noah find her, and the three run into Kieran, who followed Emma to the park. Brooke lures Branson to a hotel room and threatens to kill him if he does not confess to killing Jake, to no avail. Believing him but still upset, Brooke leaves him handcuffed to the bed. Audrey tells Noah about her connections with Piper: Audrey wanted Piper's help to make a documentary about Brandon James but when people started dying, Piper lied and proclaimed her innocence, which made Audrey blame herself for the murders. She tells Noah everything about the texts and the storage room and asks Noah to let her tell Emma herself. Unbeknownst to Noah and Audrey, their whole conversation was accidentally recorded. Kieran goes to Sheriff Acosta to confess the incident at the amusement park. The killer enters the room where Branson is still handcuffed, cuts off his hand, and then cauterizes the wound with an iron.

Emma is mad at Kieran for talking to Sheriff Acosta about her behind her back and Eli takes advantage of the situation to take Emma to an abandoned house and try to make a move on her. Meanwhile, Brooke thinks that Branson is texting her and threatening to expose her misdeeds in the hotel room, when it is really the killer who texts Audrey to come to the school alone or else he will expose her to Brooke. At the school, Audrey and Brooke discover the unconscious Miss Lang, who if not for the school's janitor would have been finished off by the killer. Also, Maggie and Acosta apparently have a secret regarding Brandon James and events at the farmhouse when Emma was a child. Zoe discovers Audrey's secret on Noah's podcast and e-mails it to an unknown person. The killer sets the abandoned house on fire, forcing Eli and Emma to flee. Both are unaware that a barely alive Branson is trapped inside the home with Eddie's corpse, about to burn to death.

At the annual Lakewood carnival, Audrey tries to work up the courage to tell Emma about her involvement with Piper. Brooke gets drunk before her speech for the Lady of the Lake pageant and confronts the entire town about pretending there is not a killer on the loose, and Zoe wins the pageant. Zoe and Noah are close again to being intimate when she decides he is not ready. Audrey begins suspecting Eli as the killer after a fight between him and Kieran leads to Emma telling Audrey about a restraining order against Eli by a girl in Atlanta. The killer abducts Kieran off-screen and takes him to the house of mirrors before luring Emma there. Once inside, Deputy Dwayne, who arrived to help Emma, is thrown into a mirror and killed while Emma gets his gun to shoot the killer. However, the killer uses Kieran as a decoy, who is almost shot by Emma and Acosta before his mask is taken off. He then tells Emma that the reason his stepfather went off the road, killing him and Kieran's mother is because Kieran was distracting him by arguing with him in the car. Meanwhile Zoe's email is revealed to have been sent to Emma, who learns the truth about Audrey right before she was going to tell her. Also, Maggie and Acosta continue to keep their secret while Acosta begins seriously suspecting his son is the killer.

Emma is furious with Audrey after learning that she brought Piper to Lakewood, cutting all ties with her. Following a confrontation with Audrey, she and Noah unsuccessfully try to figure out who sent the email to Emma, leading Audrey to suspect Zoe. Zoe swears that she only sent the conversation to herself and, after listening to it, deleted it. Audrey disbelieves Zoe but Noah comes to her defense and tells Audrey that she needs to talk to Emma. Meanwhile, Emma and Kieran investigate Ms. Lang, who apparently has an obsession with Emma and grew up with Piper at an orphanage. Elsewhere, the killer throws a party at the orphanage, signing it off as Emma and Audrey's. After Emma learns that Haley is involved with the party, Haley tells Emma that she is just helping out "a very special friend" whom she is seeing. Also, Stavo learns that his dad suspects him as the killer, and he finds a suspicious texting conversation between Jake and Brooke's dad. It is revealed that Haley's "very special friend" is the killer. While the killer stabs Haley to death, Emma and Audrey find Piper's long dead corpse. The cops break up the party and find Haley's corpse hanging in front of a banner that says "Face the mask".

Emma interrogates Audrey about why she brought Piper to Lakewood. Audrey tells her she did not know that Piper was the killer, but she knew that Piper was Emma's half sister. Off-screen, the killer kidnaps and buries Zoe alive, then lures Noah to the woods with Zoe's phone, before burying him alive as well. Emma and Audrey are then forced to put their differences aside in order to save Noah and Zoe. Meanwhile, flashbacks reveal that Maggie and Sheriff Acosta hid the injured Brandon James, who was still alive after his gunshot at the lake, but they do not know where he is now. In the coffin, Noah suffers traumatic hallucinations of Zoe. Emma and Audrey figure out that they need to look behind the murder board, where they find a clue that leads them to a barn by Wren Lake. They manage to dig Noah out of his coffin but when they find Zoe's she is already dead, leaving Noah distraught. The police and Maggie arrive to investigate where Noah was buried and Zoe's body is taken away. In bed, Emma confides in her mother, that they could have saved Zoe. Maggie puts a note in the tree in Brandon James's yard, telling the killer to leave Emma alone. Eli watches Maggie.

The killer sneaks into Emma's room. He takes her diary and flees before Emma wakes up. She finds a little wooden heart with the name "Emma" written on it. She wakes her mom and calls the police. The gang visits Noah at the hospital, where he is grieving Zoe's death. He tells them that his obsession with the killer is over and he will be shutting down "The Morgue". He asks them to go to his house and clean off the murder board. There, Emma finds a news article on Will's funeral and notices Eli standing in the background. This leads them all to strongly suspect Eli is the killer. Stavo visits Noah and ends up convincing him to continue "The Morgue". Sheriff Acosta questions Ms. Lang, who incorrectly says that Emma is obsessed with Piper. Eli leaves an anonymous message on Mayor Maddox's desk telling him to meet him at the barn. The killer murders the mayor at the barn and Emma and Audrey find themselves in police custody after being framed for his murder, along with every other murder the killer has committed.

The killer causes the police car containing Emma and Audrey to crash, kills the driver, and helps the girls free themselves, who take the driver's gun. He later calls them and tells them someone they love will die if they are recaptured by the police. Brooke, Noah, and Kieran join them at the movie theater. The killer arrives and stabs Brooke; she is taken to the hospital while Emma flees to avoid the police but Audrey has disappeared. The killer tells Emma that Audrey is at the orphanage where Piper grew up. Emma reunites with Audrey, and they are joined by Kieran and Eli. The boys accuse each other of being the killer. Emma realizes Kieran is the killer after he mimics a phrase the killer used earlier; he confirms this by shooting Eli dead with the driver's gun. He explains that he and Piper were romantically involved. He wanted Emma and Audrey to suffer for killing her. After a struggle where Audrey gains the upper hand, Emma decides to spare Kieran's life and he is taken away by the police. Maggie finds the note she left for Brandon James with blood on it and a knife pinning it to the tree. In prison, Kieran is surprised to receive a call from Ghostface, who asks "who told you you could wear my mask?"

Eight months after Kieran's arrest, the new Brandon James slasher murders him and a security guard in the courthouse. The remaining Lakewood survivors are shocked by his murder and, with Halloween being one year since Piper's unmasking, they decide to take a vacation from Lakewood and travel to Shallow Grove Island. There, Stavo, Noah (now published authors, having written a book about the murders of their friends) and their book editor Jeremy plan to research Anna Hobbs—a girl who supposedly killed her mother, brother, and the man having an affair with her mother, Reginald Whitman—in the hopes of writing a new book. Emma meets Alex Whitman, a descendant of the man killed by Anna, and the two take a romantic interest in each other. After a party, Stavo hits on a woman named Billie to make Brooke jealous. Billie and a store clerk named Sid are murdered by someone wearing an Anna Hobbs mask and wielding shears. Jeremy disappears after the party. Emma finds Billie's corpse when the killer calls her using the Brandon James voice, gloating she will never be rid of him.

The gang unsuccessfully attempts to escape on Alex's boat, which has been stolen. The town's sheriff is also killed and his severed head is placed on the doorstep of Alex's mansion. After Jeremy returns, alive and unscathed, he is locked in a closet by Alex, as everyone believes he is the killer. However, he escapes and discovers a picture of Billie and the real Alex Whitman, revealing the Alex they know is an imposter and the killer; Alex, whose real name is Tom Martin, then kills Jeremy. Noah, Audrey, Brooke, and Stavo travel through a secret passageway from the mansion to Billie's house. There, Noah deduces that instead of Anna being the killer, she and her family were attacked by Reginald and he was only killed by Anna in self-defense after stabbing and attempting to rape her. Anna later died from her injuries. Emma is briefly attacked by Tom in his costume, but she locks him out of her room. Tom takes off the costume and gets her to let him in the room with her by pretending to be Alex. However, Emma discovers the corpse of the real Alex and realizes that Tom is the killer. Tom explains that his parents were murdered and he, like Emma, is a survivor and he felt it necessary to kill anyone standing in the way of a relationship between the two after seeing her on the news. Tom also reveals he did not kill Kieran and does not know who did. When Emma explains that she does not love him, he chases her until she shoves him off the balcony of the mansion to his death. Back in Lakewood, as Noah questions on his podcast who killed Kieran, Emma's father, Kevin, is shown standing at Kieran's grave and a man named "Mr. James" checks into a hotel.
