A screened-in porch was tossed over a house, large hay bales were rolled many yards (some into a road), and several trees were downed. The tornado was observed by several local residents.[15]

A single-story home was swept off its foundation, and the second floors of several two-story homes in town were either completely removed or sustained significant damage. A well-anchored mobile home was thrown 50 yards (46 m) into a tree, vehicles were rolled, and a large chemical tank was thrown 100 yards (91 m). In Osage County, a farmstead sustained damage and trees were downed sporadically until the tornado dissipated. Five people sustained injuries in Lyon County.[16][17]

EF1 Northern Minneapolis to NE of Fridley Hennepin, Anoka, Ramsey 44°57′N 93°22′W / 44.95°N 93.36°W / 44.95; -93.36 (Minneapolis (May 22, EF1)) 1915 14.25 miles (22.93 km) 1 death – Large wedge tornado caused high-end EF1 damage throughout northern Minneapolis and surrounding suburban areas. Many trees and power lines were downed, while roofs, sheds, and garages sustained significant damage. One man was killed when a tree fell onto his car. In an industrial area of Fridley, numerous large warehouses were damaged, including some that lost their roofs. 8 train cars were blown over as well. Several hangars were damaged at the Blaine-Anoka County Airport before the tornado dissipated. A second person died after suffering a heart attack while clearing debris in the wake of the tornado. Damage from the tornado was estimated at $166 million and 48 people were injured.[22][23]

Numerous 100-foot tall and 3-foot diameter trees were either uprooted or snapped off as well. Minor roof damage and extensive tree damage occurred as the tornado clipped the south side of Hokah.[26]
Iowa

The worst damage occurred near Chester. In Minnesota, structures had their roofs torn off and silos were damaged. 4 people were injured.[27]

Wisconsin

EF1 SW of Brill Barron, Washburn 2100 2 miles (3.2 km) Roofs were torn off of a log house and farmhouse. A 7-foot house antenna was downed and two horse enclosures were destroyed. Extensive tree damage in the area as well.[34]
EF2 La Crosse La Crosse 43°50′N 91°14′W / 43.83°N 91.23°W / 43.83; -91.23 (La Crosse (May 22, EF2)) 2123 2.3 miles (3.7 km) An ice arena sustained significant damage at the beginning of the path, and numerous trees were snapped. Homes sustained major roof and garage damaged, and industrial buildings were severely damaged as well. An apartment building had its roof torn off. 200 homes and businesses were damaged, 9 homes were destroyed and another 6 buildings were left uninhabitable. First tornado in La Crosse since 1966.
EF1 E of Rockland to Sparta La Crosse, Monroe 2147 2 miles (3.2 km) A few trees and buildings were damaged at the beginning of the path. A house and four mobile homes were destroyed further along the path before the tornado struck Sparta, where 50 homes were damaged. At a car dealership on the south side of Sparta, a car was tossed 40 feet and flipped upside down and a billboard sign was knocked over on to a truck.[35]
EF2 NW of Tomah to SE of Plover Monroe, Juneau, Wood, Portage 44°09′N 90°18′W / 44.15°N 90.30°W / 44.15; -90.30 (Tomah (May 22, EF2)) 2215 69 miles (111 km) Very long-lived tornado tracked through mostly wooded areas of Wisconsin, snapping or uprooting thousands of trees and power poles, some of which landed on homes. A house and a mobile home were destroyed at the beginning of the path, and a third home was heavily damaged. A cranberry farm sustained major damage near Finley, and a campground was damaged near Nekoosa. Homes were also heavily damaged near Wisconsin Rapids. Numerous silos, irrigation systems, and outbuildings were destroyed along the path as well. Tornado was on the ground for 1 hour and 31 minutes. Ranks as the 15th longest tracked tornado in Wisconsin history.[36]

EF0 SE of Waverly Lafayette 2025 6.5 miles (10.5 km) Tornado made several touchdowns along its path, removing the roof off one mobile home and downing trees. A garden shed was damaged as well.[45]
EF1 SW of Slater Saline 2148 8 miles (13 km) High-end EF1 tornado made several touchdowns along its path, destroying one house and tossing a single-wide mobile home 50 yards (46 m). Minor outbuilding damage occurred as well.[46]
EF5 SW of Joplin to ESE of Diamond Newton, Jasper 37°03′09″N 94°35′36″W / 37.0524°N 94.5932°W / 37.0524; -94.5932 (Joplin (May 22, EF5)) 2234 – 2320 21.62 miles (34.79 km) 158 deathsSee article on this tornado – The tornado began in Newton County at EF2 intensity before continuing into Jasper County and producing catastrophic EF4 to EF5 damage in Joplin. The tornado reentered Newton County, weakening back to EF2 strength before dissipating east of Diamond. An estimated 1,150 people were injured.
EF2 E of Diamond to N of Monett Newton, Lawrence 37°02′N 94°14′W / 37.03°N 94.23°W / 37.03; -94.23 (Wentworth (May 22, EF2)) 2310 17 miles (27 km) This large wedge tornado touched down as the Joplin EF5 tornado weakened. Minor tree and roof damage occurred at the beginning of the path before the tornado intensified and destroyed three mobile homes, one of which was wrapped around a tree. Numerous trees and power poles were downed nearby. The tornado struck Wentworth at EF1 strength, removing the roof from an outbuilding and downing numerous trees in town. The tornado regained EF2 strength past Wentworth, where a mobile home was wrapped around a building and a machine shop was partially destroyed. Trees, fences, frame homes, and outbuildings were damaged at EF1 strength further to the east before the tornado dissipated.[47][48]
EF2 NW of Galena Stone, Taney 36°50′N 93°30′W / 36.83°N 93.50°W / 36.83; -93.50 (Galena (May 22, EF2)) 0010 16 miles (26 km) Several turkey barns and two mobile homes were destroyed. A few homes sustained roof and siding damage and outbuildings were destroyed. Numerous trees were snapped or uprooted along most of the track. Illinois
EF1 E of Forreston Ogle, Winnebago 42°07′N 89°32′W / 42.117°N 89.533°W / 42.117; -89.533 (Forreston (May 22, EF1)) 2325 29 miles (47 km) In Ogle County, two mobile homes were heavily damaged, and multiple power poles were snapped or pushed over.[54] In Winnebago County, a middle school had part of its roof blown off, numerous trees were downed, homes sustained minor damage, and outbuildings were destroyed with debris scattered up to 400 yards away in some cases.[55]
EF1 S of Polo Ogle 41°58′N 89°34′W / 41.967°N 89.567°W / 41.967; -89.567 (Polo (May 22, EF1)) 2328 2 miles (3.2 km) Several farm buildings were damaged, including a grain bin which was ripped from its foundation. 2x4's were found speared into the ground and insulation was plastered against nearby buildings. Shingles were torn from roofs, and multiple trees and power poles were snapped. Several cattle were injured, one fatally.[56]
EF1 N of Stillman Valley Ogle 42°09′N 89°10′W / 42.150°N 89.167°W / 42.150; -89.167 (Stillman Valley (May 22, EF1)) 2340 1 mile (1.6 km) Tornado destroyed a wood truss electrical transmission line tower, tore off the gable and part of the roof of a house, peeled soffit and shingles from a couple other houses, and knocked down trees and limbs. Truck trailers were also overturned.[57]

Oklahoma
EF2 SE of Cleora Delaware 2333 6.7 miles (10.8 km) Large stovepipe tornado caused major damage along Grand Lake. Multiple frame homes were damaged, some heavily. Seven mobile homes were completely destroyed, along with some boat docks. Numerous large trees were snapped and uprooted.[60]
EF3 WSW of Zena, OK to NW of South West City, MO Delaware (OK), McDonald (MO) 2352 16.5 miles (26.6 km) Large wedge tornado began in Oklahoma, where numerous barns, frame homes, and mobile homes were heavily damaged or destroyed. Very little debris was recovered from some of the mobile home sites. A steel-frame building was also destroyed, and numerous large trees were snapped and uprooted as well.[61] Tornado reached high-end EF3 strength in Missouri, where a brick house was leveled and a car was rolled 200 yards. Several chicken houses were damaged before the tornado dissipated. 4 people were injured.[62]
EF1 N of Leach Delaware 0113 4.7 miles (7.6 km) Tornado damaged several homes and barns, and snapped or uprooted a number of large trees.[63]

Pennsylvania

A concrete silo was collapsed onto a barn, killing 7 cows, homes had their roofs torn off and were shifted from their foundations, and numerous sheds and outbuildings were destroyed. Many trees were snapped and uprooted along the path.[80]
EF1 W of Winfield to E of Union, Northumberland 40°54′N 76°52′W / 40.90°N 76.87°W / 40.90; -76.87 (Winfield (May 23, EF1)) 2135 3 miles (4.8 km) Several homes were damaged, some heavily. Outbuildings and a cinder-block shed were destroyed, with debris found embedded into nearby trees. Many trees were downed as well, some of which landed on recreational vehicles and a pavilion at a campground.[81]

Near , a house was damaged and a barn was destroyed with debris scattered hundreds of yards away. An irrigation pivot was overturned and two grain bins were also destroyed, including one that was thrown 265 yards. Near Grantsburg, a home sustained roof damage, a pole barn was destroyed, and swirl marks were left in grass fields before the tornado dissipated. Numerous trees were snapped or uprooted along the path.[171]
